Forms and Resources

Here are several links and information about forms that will help guide you through your graduate studies.

Do-All Form

This multi-part "do-it-all" form is used to notify the graduate adviser, the graduate division of the School of Engineering, and the KU Office of Graduate Studies of important events such as:

  • Changing majors
  • Changing from M.S. to Ph.D. status (requires a letter from the major professor)
  • Transfer of credit (requires a letter of request from the student, a copy of the student's transcript, and perhaps a course syllabus or other information describing the course content, level of instruction, etc.)
  • Certification of research skills and time-in-residence (for Ph.D. students)
  • Scheduling of M.S. final general exams
  • Scheduling of Doctorate comprehensive and final exams

A completed Do-All form must be submitted to the graduate adviser at least two weeks before an exam can be given. The course/research adviser will complete the form in coordination with the Graduate Adviser.

Blank and completed forms are kept in the CEAE Graduate Programs Office (2142 Learned Hall, 785-864-3826).
